ICS uses a fully-online athletic clearance system for all student-athletes in grades K–12. This system streamlines how we collect and manage sports documentation, helping us build a central database for all ICS athletes. You’ll use the same login every year.

We are no longer accepting paper forms. The only way to be cleared to play is through the online portal.
What you need to know:
All students (K-12) must create an account—even if they are only interested in playing a sport. Selecting a sport does not commit your child to a team but helps us gauge interest and plan accordingly.
- K–6th Elementary Teams: Only basic information and a signed ICS Athletic Elementary Parent Contract are required.
- Middle School and High School Teams the following are required:
- ECG Screening Form (EL01). Required for all incoming 9th grade students and new HS students to ICS.
- EL2 Form (must be printed, completed by your student’s doctor, and scanned into Athletic Clearance). A new EL2 form is required each year and is valid for one year from the date of the physical.
- EL3 Form (viewed online and digitally signed)
- ICS Athletic MS-HS Parent-Student Contract (digitally sign, and submit it online) this will be reviewed over in detail during the parent meeting for each sport.
- Waivers if applicable (sport specific, view and digitally sign online)
- NFHS Learn Video Requirements must be completed each year after May 15th. The NFHS account you complete them on must be in your student’s name. Then, you must upload the certificates into Athletic Clearance.
